How do you fill in a service voucher?
For every hour of work done by your household helper, you give him/her a service voucher, that you have purchased in advance from Sodexo Titres Services.
How do you fill in the service voucher?
Since 22 February 2010, the service voucher has been printed on the front and the back; you now have more room to write any comments.
It is important to fill the vouchers out legibly, in dark blue or black ink. Please do NOT use red or green ink.
The front of the voucher is to be filled out by the user, with the following information:
- date of the service
- signature
- a tick to be put to the activity box
The back is to be filled in by the approved service-voucher company Aaxe "titres-services", with the following information:
- approval number of the company
- name of the approved company
Your household helper will add the following information:
- his/her social security number (NISS)
- surname and first name
- signature.
